The Aldersbridge Navigator is a distinct, community-based resource. We have created a professional resource which can reach into the community and help identify individuals at risk, bring the necessary resources in order to keep individuals safe in their primary residences for as long as possible. The Linn Health Navigator is a professional education and referral resource for the community.

We focus on identifying and addressing the needs of some of the most vulnerable seniors in the community. It is well understood that a very small percentage of seniors consume a disproportionate share of expensive medical care – such as unnecessary hospital readmissions – due to uncontrolled or poorly managed chronic conditions like diabetes, congestive heart failure, polypharmacy and many others.

 

Because of the Navigator’s position in the community, we can identify these individuals in advance, connect them with the preventative medical, healthcare and, community services that they need – keeping them as healthy as possible for as long as possible.
